I seem to remember filling out paperwork where I could request a hearing based on behaviors displayed by the custodial parent. Namely, name calling, putting the child in the middle of our dispute and what not. Does anyone know where I can get a copy of that paperwork for my records?

What form you are talking about is probably the Parenting Plan. Look in this, and see what it says for limitations on behavior/conduct with the child present. Once you find this, and are able to establish conduct is inappropriate, you can file a petition with the court.
You need to really look at if the behavior is really threatening to the child, or is it more towards you? Also need to determine if going to court to limit his parental rights until conduct improves is really worth the battle? By all means, keep your child protected!! You may also be able to resolve your issues out of court in a mediated enviroment, such as a Dispute Resolution Center.
